Step #1.

Review the following documents and then complete the Specimen-Data Request Form online.

  • Overview of Request Process - This is a step-by-step overview of the process from request submission to # request fulfillment.

  • Disbursement Committee Review Form - his form is completed by members of the Prostate SPORE disbursement committee to determine whether a request is approved or denied.

  • Request Guidelines - * Additional information regarding your project type, banked specimens, IRB approval/exemption, available clinical data fields, and more. Failure to adhere to these guidelines may result in the rejection of future requests.

    Step #2.

    Provide proof of IRB approval or exemption.

    If the project for which you are seeking specimens or data has already been approved by your IRB or if you have been granted an exemption, please proceed to step 3.

    If your project has not yet been approved or granted an exemption, please follow the instructions below.

    If your research is being conducted at Northwestern University: The project for which you are requesting specimens or data will need Northwestern University IRB approval or exemption. An exemption may be granted if you are requesting de-identified data or specimens that cannot be linked to the subject.

    To request IRB approval or exemption, follow the Clinical Research Office New Protocol Submission Process. If you would like to prepare your own IRB submission, please see the Northwestern University IRB website for guidance.

    If your research is being conducted at an outside institution: Please consult with your institution's IRB to obtain approval or an exemption.

    Step #3.

    After the IRB has approved your project or granted an exemption:

    Fax (270-479-0888) or email your letter of approval or exemption to Michelle Pattison. You will be contacted regarding specimen/data disbursement within a week.
